Make an appointment with a physician or an advanced practice registered nurse.
Complete the on-line patient intake and consent forms at least 24 yours prior to your appointment.
Once a qualifying condition has been established, we begin the application for the CT MMP during your medical visit. Your appointment time can last 15-60 minutes depending on if this visit is for a new card or a renewal of an existing card.
Connecticut requires you to set up a patient Biznet Account to apply for your license. For this, you will need a scanned copy or photo of your driver’s license (or passport, etc.) and a scanned copy or photo of what is called “proof of residence” This is in the form of a bill that has your name on it and your CT address. The cost to the state is $100. This is done online with a charge card or bank card.
Your permanent card (last 1 year from your medical visit) will arrive via email in approximately 3 weeks, at which time you can schedule a consultation with a pharmacist at the dispensary to discuss medication options.
Yes, you need one in the state of Connecticut. You can ask your doctor to certify you as a patient in the medical marijuana program if you have one of the qualifying conditions recognized to benefit from medical cannabis.
No, at this time medical marijuana is not covered by health insurance due to cannabis still being federally classified as a schedule 1 controlled substance.

If you choose a caregiver to be on your Biznet account-$25.00 State Annual Fee
We take credit card and debit cards for payment at this time. Insurance does not cover a medical visit just for cannabis and obtaining a MMP card at this time.
We accept debit cards, credit cards and checks. We will send you a method to pay online.
